6/2/2023 EnchantaVaughn Approved (AM) Upon arrival, client appeared to be in a standoffish mood as evidenced by his closed body language and looking away when greeted by QMHP. Teacher reports client displays poor relationship building skills in class and is starting to get off task again. Client was seen displaying inappropriate social behaviors (i.e. touching other peer belongings and taking things that donât belong to him) during group activities. Client appeared to be in a content mood, as evidenced by his polite expressions and great eye contact. Client expressed he was going to have a good day but couldn't hold up his word. As soon as he was faced with challenging tasks, his positive behaviors switched to negative behaviors going into lunch. (PM) Client displayed poor decision making skills by refusing to listen to his teacher and attend specials. Client was observed leaning on the wall outside of his class and not complying to authority requests and demands. Client stated he didnât want to be in the room and he was not interested in any of the assignments. When client realized his only options were to go into the class or get a 0, he was not phased by the consequences and tried to leave out of the room without permission. Client did not want to go back to class. Client chose to display aggressive tone and disrespectful attitude with his counselor when redirected. (EOD) Client displayed defiant behaviors and avoided conflict management. Client is often shying away from expressing himself and struggles with verbal communication. Client exhibited low self control when he is put into an environment that gives him more structure, as evidenced by his impulsive and uncooperative behaviors. Client did not appropriately communicate his feelings and attempted to walk away when no one noticed him. Client was reminded to use his words and to be patient. Client became a little frustrated when having to ask for permission due to the relationship he has with one of his teachers. Client left recess to go sit in his classroom but had trouble transitioning back to the academic setting, as evidenced by him pushing his desk forward and kicking his chair. Client displayed defiant and aggressive behaviors, refusing to listen to teacher commands. Client failed to meet objectives 1, 2, and 3.
